Home & Community Based Services

We provide a comprehensive array of supports to children on the Intellectual Disability, Brain Injury, and Children’s Mental Health Waivers, as well as teens and young adults through home-based habilitation services.

Intellectual Disability and Brain Injury Waiver Services
We offer both supported community living and respite services for children on the Intellectual Disability and Brain Injury Waivers. If your child is not on a Waiver, but you think he/she qualifies, contact Tony

In supported community living services children work on a wide array of skills on an individualized basis to increase their independence in our community. This includes:

  • Personal hygiene
  • Money management
  • Social skills
  • Communication skills
  • Leisure activities
  • Public transportation
  • Nutrition
  • Meal preparation
  • Household skills
  • Volunteering in our community

In respite services, the primary caregiver of a child on the Intellectual Disability or Brain Injury Waiver is provided with a break from the day-to-day care of their child. Our qualified respite providers primarily give evening breaks, but weekend respite is available on a scheduled basis. We have a number of respite providers who are family or friends of the client, as well as regular HCBS staff who provide respite.

Children’s Mental Health Waiver Services
We provide a comprehensive array of services to children, teens and their families on the Children’s Mental Health Waiver. If you think your child qualifies for the Children’s Mental Health Waiver and you would like support in applying, contact us today.

In-home family therapy is a skilled therapeutic service with a licensed therapist to increase the child's and family’s ability to cope with the effects of a serious emotional disturbance and maintain a cohesive family unit.

Family and community support services
assist the child and family by developing and implementing strategies and interventions that reduce stress and depression, and increase social and emotional strength. This includes services for the child and family members, as well as medication management, personal hygiene, positive socialization and citizenship skills.
